Licensed Practical Nurse (LPN) Home Care

Position Summary: Join our compassionate team as a Licensed Practical Nurse (LPN), providing essential in-home care to clients while working under the supervision of a Registered Nurse (RN). You will implement nursing care plans developed in line with physician's orders, offering you the unique opportunity to make a difference in patients' lives.

Location: Baldwin County, Point Clear, AL - Monday to Friday, 8 AM to 4 PM

Essential Duties

• Perform a range of nursing procedures including tracheostomy care, colostomy care, catheterizations, dressing changes, and more.
• Educate patients and families in accordance with the RN's care plan.
• Monitor vital signs and perform specific measurements, reporting significant changes promptly.
• Document patient intake and output; observe and record physical and mental conditions.
• Administer medications as instructed, ensuring accurate documentation.
• Collaborate with patients, families, and healthcare team members to coordinate care effectively.
• Maintain confidentiality and uphold regulations, including Medicare, Medicaid, and HIPAA.
• Demonstrate consistent attendance and flexibility to meet agency needs.
• Perform additional duties as required to support patient care.

Position Requirements & Competencies

• Graduated from an accredited practical nursing program
• Licensed as a vocational/practical nurse in the state
• Minimum of two years clinical experience in practical nursing
• Comfortable making home visits and providing care in diverse environments
• Proficient documentation skills; able to complete paperwork legibly and timely
• Strong interpersonal and communication abilities, both verbal and written
• Reliable transportation, current driver's license, and appropriate automobile insurance
